This page gives sample email forms that you can copy and use.

The benefits of having a form over the use of mailto: links are that you get a higher response, and they are more secure, see Review of Mailto: Encoders.

Of course you would need to fit these form into your site look and feel, ideally through picking up your existing CSS style sheet.

Not actually a form, and using a cgi script instead of formmail, but quite useful, EXCEPT that it relies on the presence of a configured email client such as Outlook Express on the user's PC.

It does not work well with users of web based email, who have to copy and Paste the To: email address.

Simple Enquiry Form

Probably the best for most purposes. Sends an email direct to the system administrator or whomever, without relying on a user email client.
